Chapter 5 - The Woman Who Changed the Chart

For several seconds nobody moved.

Rachel looked toward Lily.

The baby slept beneath a thin hospital blanket, unaware that three adults were listening to the dead woman's voice trying to protect her.

Grant stopped the recording.

"What chart?"

Naomi opened another folder.

"Melissa never found out."

Grant stood.

"We find out now."

The hospital resisted.

At first.

A records supervisor said privacy regulations prevented immediate access to internal modification logs.

Grant reminded her he was Lily's father.

Naomi reminded her that a forged authorization concerning Lily was already under investigation.

Rachel reminded her that someone had nearly dropped a wooden picture frame onto Lily less than twelve hours earlier.

The supervisor made a call.

Within forty minutes they were sitting in a conference room with the hospital's chief medical officer, legal counsel, and information security director.

Paul Danner was not invited.

That mattered.

The audit log appeared on a screen.

Lily's electronic chart had been altered seven times by administrative users who were not part of her clinical team.

Four changes were routine insurance corrections.

Three were not.

The first change added Vanessa as an emergency contact.

Grant frowned.

"I never approved that."

The second uploaded the forged temporary medical authorization.

The third changed a note written by one of Lily's nurses.

The original note read:

Mother deceased. Father retains sole parental authority. Stepmother Vanessa Hart has requested unsupervised access. Father has not authorized independent medical decision-making.

The altered version removed the final sentence.

Rachel stared at the screen.

"Who changed it?"

The information security director hesitated.

Naomi leaned forward.

"Name."

"Paul Danner."

Grant closed his eyes.

The chief medical officer looked sick.

Danner had used his administrative credentials.

He had also accessed Lily's records fourteen times in five days.

Nine of those accesses occurred outside normal working hours.

"Why?" Grant asked.

Nobody answered.

Rachel looked at the log.

"Who was the nurse who wrote the original note?"

The hospital counsel checked.

"Claire Dawson."

"Where is she?"

Another hesitation.

"She was placed on administrative leave yesterday."

Grant's head snapped up.

"Why?"

"Patient confidentiality complaint."

"Filed by whom?"

No answer was necessary.

Rachel knew.

"Vanessa."

The counsel looked down.

Rachel almost laughed.

The pattern was becoming obscene in its simplicity.

Anyone who questioned Vanessa became unstable.

Dangerous.

Unprofessional.

Disloyal.

Just as Rachel had become violent the moment she saved Lily.

Naomi requested Claire Dawson's contact information.

The hospital refused.

Grant stood.

"Then call her yourselves."

The chief medical officer agreed.

Twenty minutes later Claire joined the meeting by video.

She looked exhausted.

She was thirty, with dark hair tied back and the careful expression of someone who had spent the previous day wondering whether her career was over.

Grant introduced himself.

Claire immediately said, "How is Lily?"

That told Rachel everything she needed to know about the nurse.

"Stable," Grant said. "Why did you write that note about Vanessa?"

Claire hesitated.

Naomi explained that the call was being documented.

Claire took a slow breath.

"Because Mrs. Hart kept asking to be alone with the baby."

Vanessa still used her maiden surname professionally, even after marrying Grant.

Grant leaned forward.

"Why?"

"I don't know."

"Did you let her?"

"Not after the first time."

Rachel felt her body stiffen.

Grant's voice lowered.

"The first time?"

Claire nodded.

"It was the night after Lily's abdominal procedure."

Grant looked at Rachel.

Rachel remembered that night.

Grant had gone home for three hours to shower and change clothes.

Rachel had arrived later.

Claire continued.

"Mrs. Hart said you had authorized her to sit with Lily while you rested. The chart listed her as an emergency medical decision-maker. I assumed the paperwork had been verified."

"What happened?"

Claire's expression tightened.

"Nothing obvious."

"That's not an answer."

"I found Lily crying. Her feeding line had been clamped."

Grant stopped breathing.

Rachel gripped the edge of the table.

Claire continued quickly.

"Only for a short period. Lily was stable. I corrected it immediately."

"Who clamped it?"

"I don't know."

"Was Vanessa in the room?"

"Yes."

"Was anyone else?"

"No."

The conference room went silent.

Grant looked toward the hospital executives.

The chief medical officer lowered his eyes.

Claire said she reported the incident.

The report disappeared from her queue the following morning.

Paul Danner told her the issue had been resolved.

Later that day, Vanessa complained that Claire was harassing her family.

Claire was placed on leave.

Rachel thought of Lily's weak cry.

Her fresh abdominal bandage.

The frame descending.

"Did Vanessa touch Lily's medical equipment any other time?" Rachel asked.

Claire hesitated.

"Once."

Grant's voice sharpened.

"When?"

"Two nights ago."

"What did she do?"

"I didn't see exactly. When I entered, she was standing near the medication pump."

Rachel's skin prickled.

Claire continued.

"She stepped away quickly."

"What did the pump show?"

"It had been paused."

Grant rose so fast his chair struck the wall.

The hospital's chief medical officer immediately ordered a full review.

Naomi did not look surprised.

She looked furious.

Then Claire said something nobody expected.

"There was another person."

Grant stopped.

"What person?"

"The first night. Before I found the feeding line clamped."

"Who?"

"I didn't know her name then."

Claire looked directly into the camera.

"But I saw her yesterday in the hallway with Mrs. Hart."

Rachel already knew.
